DSS funding standard notification:

The information that you provide on this form includes your personal information. Your personal information is protected by law, including the Privacy Act 1988. Your personal information collected on this form is used primarily for Pathways to Resilience’s DSS reporting purposes.

As part of the services provided to you by Pathways to Resilience, we need to collect some information about you to assist the Australian Government Department of Social Services to conduct performance reporting and research relating to the services that you receive from our organisation. To assist this process, Pathways to Resilience will enter your personal information onto the DSS Data Exchange web-based portal which is administered by the Department of Social Services. The Department of Social Services will not use your personal information in an identifiable form when conducting its research and evaluation, except where you have agreed or it is required by law.

1.  Privacy Notice

Safeguarding your personal information is important to Pathways to Resilience. We are committed to complying with the Commonwealth Privacy Act 1988 and the Australian Privacy Principles and all other applicable privacy laws.

2. Use of information

Pathways to Resilience collects personal information in the manner outlined in the DSS funding standard notification above.

3. Other Service Providers

Written consent to collect and share personal information with other service providers is required before our organisation will proceed in sharing information.  Written consent may be in the form of a letter, email or a completed Confidentiality and Consent to Share Information form.

4. Security

 We will protect personal information from misuse and loss:

  • by restricting access to computer systems and physical records to authorised persons only;

  • by employing fire walls and virus scanning tools to prevent unauthorised person and viruses from entering the systems;

  • by using secure networks when transmitting electronic data;

  • by implementing a ’clean desk’ policy in all of our premises in respect of physical files; and

  • by providing secure storage for physical records.  We destroy by shredding (physical files) or deleting (electronic data) or permanently de-identify personal information we no longer need.

5. Changes to Policies and Procedures

From time to time and in line with customer expectations and legislative changes, our privacy and security policies and procedures will be reviewed and, if appropriate, updated. 

6.  Complaints Resolution

We are committed to providing participants, and other parties whose personal information we hold, a fair and responsible system for the handling of their complaints.  If at any time you have any complaints in relation to privacy, please contact us as indicated below. We will seek to address any concerns that you have through our complaints handling processes, but if you wish to take matters further you may refer your concerns to the funding body.
